Our staff is happy to assist with Public Records requests. To help us process your request efficiently, please provide the location's address and, if requesting Run Reports, include the incident date. Please allow up to 10 business days for processing.

Important: Medical record requests are subject to HIPAA regulations and must provide proper documentation.
Lakeland Fire Department is a non-transport department. All medical billing and transport inquiries should be directed to Polk County Fire Rescue at (863) 519-7350.
